  • Services provided by Oak & Ember are non-medical lifestyle support only. We do not provide nursing, medical care, or therapy.
  • Scheduling is based on client needs and caregiver availability. We strive to maintain consistency but cannot guarantee specific caregivers for every shift.
  • Families are responsible for providing accurate, updated information regarding health, safety, and household needs.

💳 Payment Policy

  • Hourly Rate: $____ per hour (2-hour minimum per visit).
  • Packages: Silver, Gold, and Platinum options available for weekly or monthly billing.
  • Payment is due as agreed upon in the Service Agreement (weekly, bi-weekly, or monthly).
  • Accepted methods: Credit/Debit, Check, ACH Transfer.
  • Late payments are subject to a $25 fee after 7 days past due.

❌ Cancellation Policy

  • We require 24 hours’ notice for cancellations.
  • Less than 24 hours’ notice may result in a cancellation fee equal to 2 hours of scheduled service.
  • Repeated cancellations may affect ongoing service availability.

🚨 Emergency Procedures

  • In case of a medical emergency, our staff will immediately call 911 and then contact the client’s designated emergency contact.
  • Staff are trained to respond quickly but do not administer medical treatment.
